Parents & Players,

Enclosed is the Riverside Water Polo clothing order form. The order forms are due no later than Friday, January 16th. The order will be placed with our vendor the following Monday.

Please be sure to include the athlete’s name, phone number, e-mail, and age level on the order form. Also, please use tally marks in the areas and sizes you wish to order. Note: if you wish to order a youth size please use a “Y” instead of a tally mark. In other words a tally mark will be an adult size and a “Y” mark will be a youth size.

The money for the clothing orders are due at the time you turn your order form in. Please make all checks payable to Riverside Water Polo. Your order will not be placed unless payment is enclosed. You may return your completed order forms and payment to any of the coaches.
